RDP Concurrent Sessions patch termsrv.dll rdpclip.exe
what approach was used in this patch?
what tools would be used to begin development

where is a good place to begin to understand terrmsrv.dll

what is known about hex edits of the current patched termsrv.dll
(something with matchin windows build version perhaps)

I take it from below rdpclip is non critical to basic rdp function

File Copy is an extension to Terminal Services server that allows you to copy and paste between server and client.

It is associated with Remote Desktop Use
Only needed if you allow remote desktop connections to your computer. If not, it is ok to disable.
Runs if you are connected to your desktop via Citrix or Remote Desktop
RDPCLIP.EXE is part of the Remote Desktop functionality. When enabled, this service allows for incoming connections via Port 3389. If you do not want to enable remote connections, disable the option from the Remote tab within the System dialog found in the Control Panel or Properties when right clicking on My Computer.

When active it seems to prevent services which are ticked as "can interact with desktop" from interacting with desktop.

RDPClip is essential to copy/paste function between a remote desktop and your local machine.
rdpclip has the potential of becoming a runaway process and hanging a remote session, The service does not restart upon reboot if stopped via Task Manager.
